Accessibility Issues With Survey

Hi,
I had a client that uses a screen reader test a survey I created and below are the issues they found. Please note that the screen reader used was JAWS. This is the most popular screen reader and is used by tons of people and is critical for our clientele.

I tried to test this form for you. I was able to enter the information required on the first page and then I clicked on next.

The second page does not read and would not allow me to answer the 2nd question that states Are you disable, Yes or No

It froze after that, I could not skip and move on.
