# Displaying multipe questions in the same screen?

We want to ask for a 5-star rating (mandatory) AND a comment (optional) on the SAME screen.  
So the user can answer both by immediately clicking SUBMIT.  
They do not have to progress to a second screen to find the submit button.  
Is this possible?

I did look at the chat surveys but they are not how we want to present the two questions on the same UI at the same time
